In a multicenter trial of over 1000 nursing home residents ≥80 years of age (65 percent with moderate or severe frailty) who were taking at least two antihypertensive drugs and had a systolic blood pressure <130 mmHg, a strategy to de-intensify antihypertensive treatment resulted in similar rates of all-cause mortality and major adverse cardiovascular events as usual care at a median follow-up of 38 months. The FDA approved updated labeling for Elevidys (delandistrogene moxeparvovec-rokl) following reports of fatal acute liver failure in nonambulatory patients. The revised labeling includes a Boxed Warning for serious liver injury, restricts use to ambulatory patients aged 4 years and older with confirmed DMD mutations, and removes the indication for nonambulatory patients. Additional updates include safety monitoring recommendations, contraindications, and a new Medication Guide. The manufacturer is also required to conduct a postmarketing study to further assess liver-related risks. A meta-analysis of 84 observational studies found that antenatal and postnatal paternal mental distress (eg, depression, anxiety, stress) were associated with poorer global, social-emotional, cognitive, language, and physical development in offspring from birth to adolescence, with a stronger association with postnatal mental distress. These findings suggest that paternal mental health issues are a risk factor for adverse developmental outcomes in children. In a randomized trial of over 140 patients with high-titer anti-cyclic citrullinated peptide antibodies (but without synovitis), treatment with hydroxychloroquine (HCQ) for one year did not impact the development of clinical rheumatoid arthritis (RA) after 36 months (30 versus 33 percent) compared with placebo. Rates of inflammatory arthritis, joint symptoms, and adverse events were also similar between groups.
